The option to upload documents only appears if your case manager determines supporting documentation is needed and initiates medical verification for Disclo to manage with you and your healthcare provider.
At that time, you would get an automated email notification sent to your personal email and the upload option will appear as a "Submit documentation" button in the Actions section of your request profile. From there, you can:
Upload supporting documents you may have on hand
Enter your provider’s name and email so Disclo can send the verification form to their office directly
Make sure any documentation you upload is clear and includes the details your case manager needs to thoroughly review your request.
Documentation must include statements from your provider that substantiate the following:
Verification of condition: Confirmation of a disability, health condition, or impairment that substantially limits one or more major life activities (a diagnosis is not required).
Functional limitations: Description of how your condition creates specific limitations (functional limitations/restrictions).
Impact on job performance: How these limitations affect your ability to perform daily job tasks/duties.
Accommodation details: Specific information about the accommodation you are requesting or that your provider recommends (for example, the adjusted schedule type or the specific changes needed).
Provider’s rationale: Why the requested accommodation(s) are recommended.
Timeframe: The expected frequency/duration of the accommodation (e.g., temporary, episodic, permanent, or the anticipated start and end dates).
Provider identification: Name, NPI/credentials/title, specialty (if relevant), contact info, signature and date.
