How your personal/medical/health information is handled via Disclo.

Your medical form and any documentation are not shared with your case manager or employer. These materials are visible only to Disclo’s internal Accommodations Team, who review, process, and verify documentation for medical verification.

What is shared with your assigned case manager/employer are the high-level, non-confidential details required under the ADA and similar laws so they can fairly review your request. This typically includes:

  • Functional limitations/restrictions (not diagnoses/condition)

  • Impact on essential job duties stemming from those limitations

  • Provider-supported accommodations and brief medical/professional rationale

  • Anticipated timeframe for accommodations, if applicable (start/end dates, frequency, duration)
